Prostatitis, as the inflammation of prostate gland, is a tricky issue in men. It brings many excruciating symptoms, such as pain in lower back and lower abdomen, pain between the scrotum and rectum (perineum), weak urine flow, urinate frequently and urgently, difficult urinating. If it is an acute form, it is also accompanied by chills and fever. Acute prostatitis only accounts for 5% of prostatitis cases but the symptoms are the severest. The most common type is known as nonbacterial prostatitis, and the cause is not known. It lasts for a long period of time and causes chronic symptoms. The symptoms can be improved and then recur repeatedly.

Prostatitis is often difficult to treat due to the unknown cause of the common form and other complications exist. Some men have signs of inflammation, such as white blood cells in their semen, but not the painful symptoms of prostatitis.

Chronic prostatitis, as the common form of prostatitis, is usually developed from acute bacterial infection or an unidentified nonbacterial source. Nearly 95% of men with prostatitis are believed to have the chronic nonbacterial form (also known as chronic pelvic pain syndrome, or CPPS).

Treatments for prostatitis

Antibiotics are the most commonly-used medicine for bacterial prostatitis. This form can be treated with antibiotics for one to four weeks. Antibiotics that are appropriate for treating this form include trimethoprim, doxycycline, and fluoroquinolones. However, some men do not respond to antibiotic treatment and symptoms sometimes reappear once the antibiotics are stopped. What’s worse, the inflammation may develop chronic and a long-term antibiotics is required.

The treatment for nonbacterial prostatitis is more difficult. It usually requires several different treatments which are based on the presence of certain symptoms or characteristics. The treatments include:

Pain in genitals. The chronic pain in genitals is unbearable in some cases. Nonsteroidal anti-inflammatory drugs are needed to alleviate the pain and remove the inflammation. The common pain medications include ibuprofen, celebrex (celecoxib), aspirin, and aleve (naproxen).

Alternative medicines include Diuretic and Anti-inflammatory pill can also relieve the pain as it contains herbs that have properties of improving blood and Qi flow. It also contains other herbs that can ease the urinary symptoms. Herbal supplements such as quercetin, bee pollen, bromelain/papain, and saw palmetto (Permixon), also may be helpful.

Urinary symptoms. Medication treatments that can be used to cure urinary symptoms include alpha-blockers such as alfuzosin (Uroxatral) and tamsulosin (Flomax). Dietary changes is also important to relieve the symptoms, such as cutting down on caffeine, spicy foods, and alcohol.

Skeletal muscle tenderness. Treatments for skeletal muscle tenderness include  pelvic floor physical therapy, behavior modification, oral antispasmodics.

Psychosocial symptoms. Chronic nonbacterial prostatitis often induces psychosocial symptoms such as stress, anxiety or depression. Treatments that are effective in these symptom include stress reduction techniques, cognitive behavioral therapy, and an antidepressant.
